Receive Your Certificate

If appropriate, a signed medical certificate (PDF) will be emailed to you, usually on the same day.

What is a Sick Note and Why Might You Need One?

Employers often require a doctor’s assessment if you are absent from work. Some workplaces allow self-certification for up to 5 days, while others may request medical confirmation from a doctor sooner.

Employers have a duty of care to protect both your health and the health of others.
By consulting with a doctor, it can be determined whether you are fit to work, and whether your health may impact your ability to safely perform your role.
For example, certain medical conditions might affect tasks such as operating heavy machinery or handling food, where there could be a risk to other employees or the public.

A sick note may also be referred to as a fit note or medical absence letter.

Important:
Healthworks provides private medical certificates which are not the same as an NHS ‘Med3 Statement of Fitness for Work’.
The Med3 form is only available via NHS GPs and cannot be provided through this service.

Our medical certificates:

Cover up to a maximum of 10 working days.
Can be backdated by up to 5 working days, if clinically appropriate.
Are limited to one certificate per illness episode within a 6-month period, unless further assessment is completed.
